Understanding Prostate Cancer Basics

The prostate is a small gland that is part of the male reproductive system. It sits below the bladder and surrounds the urethra, which is the tube that carries urine out of the body. The prostate produces fluid that makes up part of semen. In the United States, prostate cancer is one of the most common cancers among men. According to the American Cancer Society, about 1 in 8 men will be diagnosed with prostate cancer during their lifetime.

Prostate cancer develops when malignant cells form in the prostate gland tissue. There are different types of prostate cancer, and they grow at different rates. Some forms of prostate cancer grow very slowly and may never cause serious problems. Other types grow more quickly and can spread to other parts of the body if not treated. Age is an important risk factor—prostate cancer is rare in men younger than 40, but the risk increases significantly after age 50. African American men and men with a family history of prostate cancer face higher risks of developing the disease.

The prostate-specific antigen test, commonly called the PSA test, measures levels of a protein produced by the prostate. Higher PSA levels may indicate prostate cancer, but they can also result from other conditions like benign prostatic hyperplasia (enlarged prostate) or prostatitis (prostate inflammation). Understanding what PSA levels mean is important for making informed decisions about your health. A PSA level below 4.0 nanograms per milliliter (ng/mL) is generally considered normal, though this can vary based on age and individual factors.

Practical Takeaway: Knowing basic facts about the prostate and how cancer develops helps you understand screening decisions and conversations with your doctor. Consider learning your family medical history, as this information is valuable for discussing your personal risk with a healthcare provider.

Risk Factors and Who Should Know About Screening

Several factors can increase the likelihood that a man will develop prostate cancer. Age is the strongest risk factor—men over 65 are diagnosed more often than younger men. Race and ethnicity also play a role. African American men have higher rates of prostate cancer diagnosis and tend to be diagnosed at younger ages and with more advanced disease compared to white men. Men of Scandinavian descent also have higher rates. However, prostate cancer occurs in men of all racial and ethnic backgrounds.

Family history is another significant risk factor. Men whose father, brother, or son had prostate cancer face higher risk themselves. If multiple family members were diagnosed, or if diagnosis occurred at a young age, the risk increases further. Genetic mutations, including changes in the BRCA1 and BRCA2 genes, are associated with higher prostate cancer risk. Men carrying these mutations should discuss screening options with their doctors.

Lifestyle factors may also influence prostate cancer risk, though research is ongoing. Some studies suggest that diet high in red meat and dairy products may increase risk, while diets rich in vegetables and fruits may lower it. Obesity has been linked to higher risk of more aggressive prostate cancer. Physical activity levels, smoking status, and sexual history have also been studied as potential risk factors, though results vary.

Major medical organizations provide different guidance about screening. The American Cancer Society recommends that men have the opportunity to make an informed decision about screening beginning at age 50, or age 40-45 for those at higher risk. The U.S. Preventive Services Task Force recommends discussing the benefits and risks of PSA screening with men ages 55-69, and notes that screening is not recommended for men over 70. These different recommendations reflect ongoing scientific discussion about screening value.

Practical Takeaway: Create a timeline of your family's medical history and discuss it with your doctor. This conversation helps determine when screening might be appropriate for you personally, rather than following a one-size-fits-all approach.

Screening Methods and What Tests Involve

The PSA test is the most commonly used screening tool for prostate cancer. This is a simple blood test that measures prostate-specific antigen levels. Your doctor draws a small amount of blood, usually from a vein in your arm, and sends it to a laboratory for measurement. Results typically come back within a few days. The PSA test can be performed in a doctor's office, clinic, or hospital laboratory. There is no special preparation needed before the test, though some research suggests that recent ejaculation, vigorous exercise, or certain medical procedures may slightly raise PSA levels temporarily.

The digital rectal examination (DRE) is another screening method. During this brief procedure, a doctor inserts a gloved, lubricated finger into the rectum to feel the prostate gland. This allows the doctor to assess the prostate's size, shape, and texture, and to feel for any abnormalities. The examination takes only a minute or two and causes minimal discomfort for most men. Some doctors perform this test along with PSA screening, while others may use it only when PSA results are abnormal or when symptoms suggest prostate problems.

Newer screening tests are being developed and studied. The PSA density test measures PSA level relative to prostate size. The free PSA test determines what percentage of PSA is "free" versus "bound" to protein—men with prostate cancer tend to have lower percentages of free PSA. The PSA velocity test tracks how fast PSA levels rise over time. Some of these additional tests may provide more information when PSA results are in a borderline range, helping doctors decide whether further testing is necessary.

If screening tests suggest possible cancer, additional imaging and diagnostic procedures may follow. A transrectal ultrasound uses sound waves to create images of the prostate. A prostate biopsy involves collecting small tissue samples from the prostate using a needle, usually guided by ultrasound. These samples are examined under a microscope to look for cancer cells. Understanding what each test involves helps you prepare mentally and physically for the procedure.

Practical Takeaway: Before any screening test, ask your doctor what to expect during the procedure, how long results will take, and what results might mean. Having these details beforehand reduces anxiety and helps you make informed decisions.

Understanding Test Results and What They Mean

PSA test results are reported in nanograms per milliliter (ng/mL). A result below 4.0 ng/mL has traditionally been considered normal, though some experts now recognize that PSA levels should be interpreted individually based on age and other factors. Men under 50 may have lower normal ranges than older men. A single elevated PSA result does not mean you have cancer—it simply means your doctor may recommend additional testing or monitoring.

If your PSA level is elevated, your doctor may recommend repeating the test to see if the elevation persists. PSA can be temporarily elevated due to recent ejaculation, vigorous cycling, prostate infection, or urinary tract infection. Waiting several weeks before retesting allows these temporary factors to resolve. If PSA remains elevated on repeat testing, your doctor may recommend additional imaging or a prostate biopsy to determine whether cancer is present.

If a biopsy is performed and cancer is found, the results include a Gleason score. This score ranges from 2 to 10 and describes how aggressive the cancer cells appear under a microscope. A score of 6 or lower suggests the cancer is likely slow-growing. A score of 7 indicates intermediate-grade cancer. Scores of 8-10 suggest high-grade, more aggressive cancer. The Gleason score helps doctors and patients understand how quickly the cancer might progress and informs treatment decisions.

The stage of prostate cancer describes how far it has spread. Stage 1 cancer is confined to the prostate. Stage 2 cancer is still within the prostate but may be larger or higher grade. Stage 3 cancer has spread outside the prostate to nearby tissues. Stage 4 cancer has spread to distant organs. Staging information comes from physical exams, imaging tests like CT scans or bone scans, and sometimes additional blood tests. Knowing the stage helps predict outcomes and guide treatment options.

Practical Takeaway: Request written copies of all your test results and ask your doctor to explain what each number and term means in your specific situation. Understanding your individual results empowers you to participate meaningfully in treatment discussions.
