Learn About Diabetic Foot Care and Prevention

Understanding Diabetes and Foot Health

Diabetes affects how your body processes blood sugar, and this condition can have serious effects on your feet. When blood sugar levels stay high over time, they damage blood vessels and nerves throughout your body—including those in your feet. About 1 in 4 people with diabetes will develop a foot ulcer (an open sore) during their lifetime, according to the American Diabetes Association. This makes foot care one of the most important parts of managing diabetes.

The connection between diabetes and foot problems happens through two main pathways. First, high blood sugar damages small blood vessels that carry oxygen and nutrients to your feet. When circulation decreases, your feet receive less oxygen and heal more slowly when injured. Second, diabetes can cause neuropathy, which means nerve damage. With neuropathy, you may lose feeling in your feet and not notice small cuts, blisters, or injuries that could become serious infections.

Type 1 diabetes (where the pancreas doesn't make insulin) and Type 2 diabetes (where the body can't use insulin effectively) both carry foot health risks. The longer someone has had diabetes and the less controlled their blood sugar has been, the higher their risk for foot complications. People who have had diabetes for more than 10 years face increased risk. Additionally, smoking, high blood pressure, high cholesterol, and obesity all increase the likelihood of serious foot problems in people with diabetes.

Understanding these connections helps explain why foot care matters so much. Your feet carry you through daily life, and protecting them means maintaining your independence and quality of life. Taking action now—before problems develop—is far more effective than treating complications later. Learning what warning signs to watch for and which daily habits protect your feet puts you in control of your health.

Practical Takeaway: Schedule an appointment with your doctor to have your feet examined and your circulation checked. Ask whether you have any nerve damage or circulation problems. This baseline information helps you understand your personal risk level and what prevention steps matter most for you.

Daily Foot Inspection and Hygiene Practices

Checking your feet every single day is the most important thing you can do to prevent serious problems. Many people with diabetes discover foot ulcers or infections only when they become severe because they didn't notice early warning signs. Daily inspection takes just a few minutes and can catch problems when they're still small and treatable. The American Academy of Dermatology recommends that people with diabetes inspect their feet at least once daily, ideally at night before bed.

To inspect your feet properly, find a well-lit area and sit down with your feet at a comfortable height. Look at the tops and bottoms of both feet, between your toes, and along the heels and sides. Use a mirror if you have trouble seeing the bottom of your feet, or ask a family member to help. You're looking for any changes from normal: new red areas, swelling, warmth, blisters, cuts, cracks, corns, calluses, or changes in skin color. Write down anything unusual so you can track changes over time. Pay special attention to areas that rub against shoe edges, since these spots are prone to injury.

Washing your feet daily is equally important. Use lukewarm water (test it with your hand or elbow first—not just your foot, since nerve damage may mean you can't feel if water is too hot) and mild soap. Wash between your toes and all surfaces of your feet. Dry thoroughly, especially between the toes where moisture can promote fungal infections. If your skin is very dry, apply a fragrance-free moisturizer to the tops and bottoms of your feet, but avoid putting lotion between toes where moisture should be limited.

Never soak your feet in hot water, even though this feels soothing. Soaking softens skin too much and increases infection risk. Also avoid harsh pumice stones or files, which can create wounds. If you have thick toenails or calluses, see a podiatrist (foot specialist) rather than trying to treat these yourself. People with diabetes have a much higher risk of serious infection from even tiny wounds created by do-it-yourself foot care.

Practical Takeaway: Set a daily reminder on your phone to inspect your feet at the same time each day—perhaps right before bed. Create a simple chart to record what you see, noting any changes. This consistency builds a habit and creates a record you can show your doctor at appointments.

Proper Footwear Selection and Shoe Care

Your shoes either protect your feet or harm them, making footwear selection crucial for diabetes foot health. Poorly fitting shoes cause pressure points, blisters, and ulcers that can develop into serious infections. Studies show that about 60% of diabetic foot ulcers occur in areas where shoes created friction or pressure. The right shoes reduce this risk significantly. Ideally, shoes should fit comfortably without pressure points, have cushioning to absorb impact, and provide support for your arches and heels.

When shopping for shoes, go in the afternoon or evening when your feet are slightly swollen from daily activity—this is closer to their actual size throughout the day. Always try on both shoes and wear the socks or orthotics (custom shoe inserts) you normally wear. Walk around the store for several minutes to test the fit. The shoe should feel comfortable immediately; don't count on a "break-in" period. Leave a thumb's width of space between your longest toe and the shoe end. Your heel should fit snugly without slipping, and the shoe should not pinch or rub anywhere, including the sides and back.

Specific shoe features matter for diabetic foot health. Look for shoes with a wide toe box (the front part) to avoid crowding your toes together. Avoid high heels, which concentrate pressure on the ball of your foot and your toes. Select shoes with cushioning that absorbs shock from walking. Many people with diabetes benefit from custom orthotics—special shoe inserts designed for their particular foot shape and pressure points. Your podiatrist or another foot specialist can assess whether custom orthotics would help you. Some research shows that wearing appropriate footwear reduces ulcer recurrence by up to 50% in people with a history of foot ulcers.

Beyond selection, proper shoe care extends the life of protective footwear and maintains its protective qualities. Inspect the inside of shoes before putting them on—check for pebbles, torn linings, or other objects that could cause injury. Rotate between at least two pairs of shoes so each pair can dry completely between uses. Moisture inside shoes promotes fungal infections. Replace shoes when they show signs of wear like thinning soles or compressed cushioning. Most quality shoes last 300-500 miles of walking before losing their shock absorption; for many people, this means replacing shoes every 6-12 months depending on activity level.

Practical Takeaway: Visit a specialty shoe store that focuses on diabetic or comfort footwear. The staff there have training in fitting shoes for people with diabetes and can help you find options that truly protect your feet. Ask whether they carry shoes designed specifically for diabetic foot care.

Managing Blood Sugar to Protect Feet

While daily foot care is essential, controlling your blood sugar is the foundation of foot health. High blood sugar causes the nerve and blood vessel damage that leads to foot problems in the first place. When blood sugar remains in a healthy range, you reduce the risk of developing neuropathy (nerve damage) and poor circulation. Research consistently shows that people who maintain better blood sugar control have fewer foot complications. The American Diabetes Association recommends working with your healthcare team to establish a target blood sugar range that works for your individual situation.

Blood sugar control involves several connected elements: taking medications as prescribed, eating a balanced diet, exercising regularly, and monitoring blood sugar levels. If you take insulin or other diabetes medications, taking them on schedule matters tremendously for consistent blood sugar levels. Missing doses or taking irregular amounts creates blood sugar spikes and crashes that damage blood vessels and nerves over time. If you struggle to remember medications, setting phone alarms, using pill organizers with labels, or talking to your pharmacist about options can help you stay consistent.

Diet influences blood sugar directly. Foods with refined carbohydrates and added sugars cause rapid blood sugar increases, while foods high in fiber, protein, and healthy fats create more stable blood sugar levels. You don't need to follow a restrictive diet, but understanding how different foods affect your blood sugar helps you make choices that protect your feet. Working with a registered dietitian—a food and nutrition specialist—can help you create a meal plan you'll actually follow. Many
